JOB TITLE: CERTIFIED NURSING ASSISTANT
REPORTS TO: R.N. /L.P.N. SUPERVISOR
JOB SUMMARY:
Assigned to a Resident Care Unit of the Bay County Medical Care Facility, will provide direct supportive services necessary for maintaining appropriate living conditions and general well being of elderly, handicapped and disabled persons. Provides continuous and primary resident contact. Supervision is direct, received from the Registered Nurse Supervisor and Licensed Practical Nurse on specific task assignment.
PRIMARY DUTIES AND RESPONSIBLITIES:
- Receives resident status report from supervisor and task assignment instructions.
- Gathers routine medical support data, taking and recording temperature, respiration, pulse, blood pressure and bodily function date. Charts data in a prescribed fashion.
- Carries out personal hygiene processes for Residents and assists in bathing, combing hair, shaving, brushing teeth, cleaning dentures, dressing and responding to miscellaneous requests. Participates in care of terminally ill and expired residents.
- Aids residents in accomplishing body functions, giving enemas, placing bedpans and
- Transporting to commodes upon request. Will chart body function data, obtain specimens, and care for resident colostomy needs.
- Regularly transfers residents, using lift or manually lifting from chair to bed, in and out of bath and applies spinal devices/prosthesis, splints and preventative skin breakdown equipment as directed on Resident Care Plan.
- Provides personal comfort services, repositioning, giving back rubs, assisting with exercise, walking and encouraging self help progress and assists residents with activities on and off the unit.
- Responds promptly to call lights providing residents needs, replacing linens, cleaning resident and observing for distress, abnormality or symptoms, reporting such observations to nurse as needed.
- Has continued responsibility for keeping assigned areas clean and in order, cleaning spills, straightening service areas, storing equipment and removing debris to appropriate storage area. Will continuously use safety practices.
- Prepares residents for meals; serves and removes food and dishes and assists with meals or feeds residents if necessary – Distributes drinking water and other nourishments to residents and records intake when appropriate.
- Will regularly use equipment, such as thermometer, blood pressure equipment, walkers, wheelchairs, bedding, oxygen cylinders, catheter, bedpans, and hydraulic lift.
- Regularly provides information for resident evaluation and assessment processes. Attends resident care conferences.
- Regularly summarizes work activity accomplished and will provide status information for caregivers on subsequent shifts.
- Is knowledgeable of Resident Rights and ensures resident privacy and dignity and helps ensure a safe, secure environment for all residents.
- Completes annual In-service training requirements, maintains acceptable attendance, and dresses in accordance with Dress Code Policy.
- Responsible for appropriate use of Facility supplies and equipment to minimize loss, waste and fraud.
- Maintains confidentiality of all data, including resident, employee and Facility information.

JOB TITLE: LICENSED PRACTICAL NURSE
REPORTS TO: R.N. SUPERVISOR
JOB SUMMARY:
As a Licensed Practical Nurse, serves as first assistant to the Registered Nurse Supervisor providing direct resident care for residents in a designated unit of the Medical Care Facility. Will lead supportive personnel in completion of repetitive resident care tasks, assist and participate in setting up and delivering medications. Will provide direct resident care to a level commensurate with licensed authorization. Supervision is direct, received from a Registered Nurse Supervisor.
PRIMARY DUTIES AND RESPONSIBILITIES:
- Receives procedural assignments and in turn assigns tasks to lower level personnel. Incorporates more responsible tasks to own work schedule.
- Assists in setting up and passing medications using unit dose procedures and the 5 “rights”. Will verify medication accuracy and observe/provide oral and injectables.
- Provides a variety of direct resident care, making dressing changes, applying ointment, eye drops, inserting and changing catheters, irrigations, tube feedings, suctioning, oxygen and trach care.
- Oversees, trains and assists C.N.A.s in completion of tasks, incorporating method and procedural changes as needed to accomplish the plan of care which has been developed.
- Will make resident rounds, taking/recording vital signs, observing for general well being, cleanliness, comfort and neatness. Reports unusual/irregular conditions to R.N. and/or Physician. Assesses the resident’s condition and nursing needs, prescribes appropriate nursing action and updates care plan.
- Prepares a variety of service reports, i.e. medication sheets, physician renewals, lab slips, graphic sheets, supply orders and charting on a continuing basis.
- Will inventory supplies and order needed replacements, storing in a prescribed fashion.
- Keeps service area free of debris, answers telephone and based on shift assignment may lock and unlock doors admitting authorized personnel.
- Completes transactions required in the event of resident expiration, contacting funeral director, preparing body, notifying service units, collecting personal property, etc. Will carry out and oversee cleaning/washing of equipment/bedding.
- Regularly uses medical equipment such as traction, feeding pumps, suction machines, glucometer, oxygen, airways, stethoscope and sphygmomanometer. Demonstrates competence in technical and manual skills according to scope of practice for L.P.N.s.
- Encourages resident in self-help activity and will observe safety practice and proper method, function as a resident advocate protecting the residents rights. Involves resident and family in planning for care, treatment, teaching and discharge plans.
- Functions as a leader for the C.N.A.s; delegates in a manner that contributes to the excellence of the nursing unit to which LPN is assigned.
- Participates in orientation of new personnel and assists in performance evaluations.
- Follows proper Infection Control procedures and supervises supportive staff in infection control procedures.
- Demonstrates ability to remain calm in emergency and/or stressful situations and promotes a feeling of confidence that situation is under control.
- Consistently follows the policies and procedures of the Facility.

JOB DESCRIPTION

JOB TITLE: MAINTENANCE I - GROUNDSKEEPER
REPORTS TO: EVS DIRECTOR
PRIMARY DUTIES AND RESPONSIBILITIES:
- Cleaning, maintaining, and repair of grounds equipment.
- Grounds keeping tasks, including lawn care, mowing, raking, trimming various plants, trees, and shrubs, weeding, and planting of trees, shrubs, flowers, and various plants.
- Shoveling, digging, sweeping, climbing, painting, and various tasks associated with maintaining the outside of the facility.
- All tasks associated with landscaping.
- Edging sidewalks.
- Collecting garbage and trash.
- Raking and general clean up tasks to keep the outside looking presentable.
- Applying fertilizer, insect spraying, and pest control.
- General grounds and outside maintenance and improvements.
- Performs grounds maintenance tasks such as operation of snowplows, blowers, shoveling snow and pick-up truck as needed.
- May be assigned to pick up various supplies for the Facility.
- Cleans non-medical areas of the facility, mopping, sweeping floors, vacuuming carpets, emptying waste baskets, and dusting. Will remove debris to designated storage areas.
- Periodically performs extended cleaning, removing wax, cleaning, waxing, buffing floors, and shampooing carpets.
- Will perform light maintenance tasks, changing light bulbs, fluorescent tubes, washing walls and service equipment, such as food carts, and other durables. Will make light repair on such equipment.
- Will participate in furniture moving, and receiving storage of materials entering the facility.
- Will remove garbage receptacles, emptying, and removing to designated service areas.
- Regularly monitors boiler room equipment, checking gauges, resetting in accordance with well-defined standard and promptly alert supervisor to improper operation of suspected malfunction.
- Performs routine generator checks and testing.
- Secures building, checking for secured doors, windows, directing visitors, noting and reacting to unauthorized visitors, and promptly alerting proper authority in the event of an emergency.
- Performs plumbing and hearing tasks, checking heating system, and augering plugged drains.
- Performs grounds maintenance tasks as assigned, will remove and gather time cards, shovel show, rakes leaves, etc.
- Regularly uses buffer, floor scrubber, mops, vacuum, receptacles, food carts, and general maintenance tools.
- Periodic assignments may include painting a variety of indoor and outdoor surfaces.
- In all capabilities utilizes safety practices to assure residents’ well-being and safety of visitors and facility personnel.
- Is knowledgeable of Resident Rights and ensures resident privacy and dignity and helps ensure a safe, secure environment for all residents.
- Completes annual Inservice training requirements, maintains acceptable attendance, and dresses in accordance with Dress Code Policy.

WORKING CONDITIONS:
- Work is performed in a resident care environment with minimal exposure to communicable diseases when using universal precaution methods. Regular exposure to and operation of various types of equipment and machinery. Occasional exposure to housekeeping/cleaning agents, chemicals, paint and other building maintenance products. Occasional exposure to outside weather conditions. Work schedule and shift may change on occasion.
- Weekend and holiday hours may be required. On-call availability to assist with emergency situations as needed.
